Some transcoded video is missing audio

Steps to replicate the bug: transcode wtv file to mp4

This does not happen a lot, but just enough to be annoying. For some reason the transcoded video for a show I have recorded using Windows Media Server (on a windows 7 vm) has no audio. The source file is fine, but the newly created file is missing the audio track.

The source is wtv converted to mp4.

I usually end up using another program to convert (sans commercial skip sadly) and it works.

Not sure what to make of it. Log of the most recent occurrence attached.

It looks like ffmpeg is having trouble processing the file while converting it
